Question: I have a 16 yo son who is always angry. He goes to bed swearing and sleeps restlessly. He wakes tired and grumpy. He treats everyone around him badly. Always irritaTed and paranoid.he constantly telling us that we infect his space. We can't place things on this side of the table or sit in his spot . If a guest comes over he makes me clean the chair ! He makes weird breathing noises like he is trying to get rid of something bad from his system. I dont know how to help him, but he is causing a lot of arguments at home but I don't want to alienate him. I'm sure he is not on drugs because I can't get him to take panda ol if he is sick... Please advise...

Brief Answer:
Likely Obsessive Compulsive Disorder, need to consult psychiatrist

Detailed Answer:
Hello XXXXXXX

Thanks for writing to us.

I can understand your concern for your son.

I read your query thoroughly. It seems that he is likely suffering from obsessive compulsive disorder(OCD).

In OCD,

Obsessions: Irrational, intrusive, repetitive thoughts come in to person's mind most of the time of the day.
Common examples: thoughts of dirt, contamination, getting infection, mistakes, symmetry etc.

Compulsions: Acts or thoughts that person has to do in response to obsessions to make himself relax
Common examples: frequent washing, bathing, cleaning, checking, avoiding some places, counting etc.

If person does not act on obsessive thoughts ,he feels too much uncomfortable.

In case of your son, he seems to have obsessive thoughts of contamination or fear of getting infected. In response to these thoughts he behaves in unusual manner like cleaning chairs of guest, does not allow to sit in his spot, weird noises to expel contamination out of the body.
He likely does not have any insight about his problem. In response to severe obsession ,he can not clean everything and also not make people to clean everything. He becomes frustrated and then abusive.

For confirmation of diagnosis , detail history, physical examination and mental status examination should be done by a psychiatrist.

Other differential diagnosis: Obsessive personality Disorder and early onset schizophrenia. They can be ruled out by psychiatrist with proper examination.

And yes , if by chance he will diagnose OCD, no need to worry much. Treatment of OCD is available in form of SSRI and anti anxiety medicines.

Hope I have answered your query, I will be happy to help further.

Regards,
