Question: I have an extremely painful red right middle toe. It deviates to the right. It has a white blister like growth toward the inside/bottom of my toe-about 1cm in circumference. I noticed a small pinlike red area to the top of the inner toe last evening. It has been hurting for some time and is getting worse. Last evening I noted my toe had become red-not warmer than the other toes. It is painful every step I take. I have partial paralysis of my right side. My great toe deviates to the right-like a hammer toe.

It would have been appreciable if you had shared the picture of the lesion, but based on the history, it seems as if it is some abscess or cellulitis which is imparting pressure symptoms on sorrounding nerves.

It needs to be reviewed urgently by a surgeon. If signs of pus or debris accumulation is found, then incision and drainage followed by antibiotic cover should be ensued. You may be asked for complete blood count and cultures of blood or pus along with uric acid levels as involvement of toe or joint swelling may sometimes point towards high uric acid levels.

Meanwhile using anti inflammatory drugs like ibuprofen will help with pain and swelling. Antibiotic like augmentin or cephalosporins should be started if fever spike is observed or inflammation progresses further.

The deviation of toe needs urgent review with the orthopedician or neurosurgeon and relieving pressure symptoms with drainage may help. Xrays of feet should be done to rule out extension of infection to the bone.
