Suggest Treatment For Extreme Tiredness While Having Diabetes

Question: I am a type 2 diabetic and lately towards the end of the work day if find myself unusually tired and lethargic. I have changed diet and have lost wgt. and have lowered my daily sugar readings. This morning it was 78 when I started my campaign to get things under control I was at 233. I was wondering if there is something I need to address.

Noted your question . Glad to know that blood glucose is currently under control. I hope your Hba1c ( three month glucose average) was normal as well. It is important to check blood glucose post meals ( 2 hour post meals) occasionally just to make sure that your blood glucose levels are normal.

I would say fasting blood glucose of 78 is too much of tight control and I suspect at times you may also be having hypoglycemia episodes . It may be good to check your blood glucose by finger prick when you feel tired. It would be great if you could provide the list of medications and a copy of your recent tests if any available.

If you are taking Metformin, it is a good idea to check your Vitamin B12 level. Metformin can lower the Vitamin B12 levels and that could be another possible cause of tiredness.

Another possibility of tiredness is hypothyroidism.

May I suggest following tests.
1) CBC, ESR
2) Urine RE
3) Hba1c
4) Renal and liver profile
5) Vitamin B12.
6) TSH,
In addition, it may be good to check multiple blood glucose readings in a day at various point of time, especially when you feel tired to rule out extremes of blood glucose readings ( High or low).
