Posted on Sun, 26 Jul 2015
Question: hi doctor i have fibula bone pain of right leg since last 15 days .i showed to a fortis doc ,he asked for an x ray first the after a week mri,Mri says i have marrow edema ,so the doc has prescribed ceftum 2 times a day and augmentin 3 times a day 625 mg,the affected area is bearably painful and i can walk but the area is mildly warm and very littlke red , kindly advice
doctor
Answered by Dr. Sunil Kumar Srivastava (17 minutes later)
Brief Answer:
Kindly post xray scan and MRI report and blood reports

Detailed Answer:
Hello
Your doctor is suspecting infection for which he has prescribed antibiotics.
However , I would like to know if there is any history of fever, trauma to leg and any recent history of infection elswhwere in past few weeks and months
Some surgeons when they suspectt infection in bone may ask for a bone scan as well which is very diagnostic of infections in bone (osteomyelitis)
If you post xray after scanning and MRI report and any other tests like CBC or blood reports it would be kind

i had a boil in right leg 2 months back which wass diagnosed as cellulitis ,and 2 years back on my left foot,,again marrow infection ,was treated with 15 day antibiotics
doctor
Answered by Dr. Sunil Kumar Srivastava (25 minutes later)
Brief Answer:
wait for antibiotics response

Detailed Answer:
thank you lil for the information
History of cellulitis may be corroborative.Dissemination of infection through bloodstream often may be a cause for bone infection
As you say antibiotics helped for left foot , trust and hope your leg problem may also settle soon.
So, wait and watch
Best wishes,

Do i have to be on bed rest?and to taking antibiotics for so long advisable? can i carry on my antidepressent fludac?

mr findings are -- early oestoarthiritis in right ankle joint
2.marrow edema in shaft of distal fibula with strain in musceles surrounding it
doctor
Answered by Dr. Sunil Kumar Srivastava (16 minutes later)
Brief Answer:
with suspected infection in bone may continue prescribed course, continue fludac

Detailed Answer:
Hi
There may be need to support the ankle with a long elastocrepe support during daytime, with minimal walking for OA ankle.
For suspected bone infections, 2 weeks or longer antibiotic courses are often prescribed and necessary also.

Fludac may be continued, shall not interfere with anti-biotics and ,if been prescribed by a physician, may be taken as advised for your other issues
Mild anti-inflammatory like diclofenac may be taken for osteoarthritis in a short course, if your treating doctor agrees
Your report of osteo-arthritis on MRI also points towards your pain in lower leg and ,in fact, co-relates with clinical symptoms
Your second report of oedema of marrow in distal part of fibula may either be due to secondary changes from OA or infection, or both( although reports no periosteal changes which happens with bone infections usually) , it may be confirmed on Bone scans, if your doctor thinks that necessary or agrees
I would prefer to confirm bone infection with bone scan if i were treating, but depends on clinical examination and your examining doctor is the best judge

Best wishes,

pl tell me can i do routine work or have to be bedridden??can this pain be because of OA?i will get bone scan done if u advise?i dont want to have antibiotics for so long

although reports no periosteal changes which happens with bone infections usually) ........i couldnt understand meaning of this sentence by u
doctor
Answered by Dr. Sunil Kumar Srivastava (23 minutes later)
Brief Answer:
periosteal (bone covering) changes do happen sometimes in bone infections

Detailed Answer:
Hi
Periosteum( its a covering of a bone cortex which gets thickened when marrow infection occurs called periosteal or cortical reaction) thickening may be seen in bone infections in x ray/MRI, although early infections may show just oedema in marrow.
At some centres , doctors ask for bone scans too for early detection of bone infection.
MRI is also specific and diagnostic and your doctor may have evaluated clinical examination, symptoms and co-related well with MRI finding to begin antibiotics
It is safe to take antibiotics as prescribed , two weeks is not a long course and you need not be bed-ridden, just be doing minimal walking ,as per necessity.
Since your doctor is reasonably sure of diagnosis, bone scan may only be an additional tool- I may say
So, trust the treatment prescribed and do not worry-you will be okay.
Yes, You may consult your doctor for Osteoarthritis treatment again.Anti-inflammatory medicine is all that is required for now, ankle support in addition

Hi doc m still in ceftum and augmentin since past 15 days , pain has completely gone , should I stop antibiotic, will it come back if I stop antibiotic

And why this is the fourth time I had infection , 2 times boil cellulitis and 2 times marrow infection
doctor
Answered by Dr. Sunil Kumar Srivastava (29 minutes later)
Brief Answer:
Antibiotics may be sttopped by your treating doctor after evaluation

Detailed Answer:
hi
nice to hear that antibiotics worked well.
if no symptoms now, it may be stopped by your treating doctor
Sometimes few persons are susceptibe to infections maybe due to low natural resistance of body.
Healthy nutrition and diet, exercises and vitamins and minerals are recommended for building body resistance and reserve.

But it's scary I might get sepsis sometime????and secondly my doc insisted I take A ti optic for 6 week but I am fed up with medication
And this time the pain started after I took massage , very strange how does infection enter body?? Can it be because I take juvederm and Botox injections once a year??
doctor
Answered by Dr. Sunil Kumar Srivastava (24 minutes later)
Brief Answer:
If doctor checked, then continue antibiotics

Detailed Answer:
thanks for query

If your doctor insists for further continuance he may have evaluated the condition.Bone infections do need long term antibiotics and depends on findings present clinically and on imaging.
Injections once a year may not be the reason, its general susceptibility -
You may see a physician also for general body check up for health ,height, weight, Basal metabolic rate, any nutritional deficiency due to any cause,Body mass index etc.For any metabolic disorder, rule out diabetes ,immunoglobins assessment-all these to evaluate for any low condition responsible for infection proneness.

But 6 week antibiotics are not dangerous ,, I am fine now
doctor
Answered by Dr. Sunil Kumar Srivastava (15 minutes later)
Brief Answer:
weigh advantages over minor risks, long term benefits

Detailed Answer:
Hi
antibiotics for six weeks may not necessarily have side-effects, but weighing advantages over manageable risks and side effects, one has to settle for greater benefit.
bone infections have notriety for chronicity and difficult to cure and need long term use of antibiotics to eradicate infections

Hi doctor , m really confused , I have few doctors my friends are telling me not to continue antibiotics, it's already 15 days I have been taking , but just for marrow edema so much medication ???
doctor
Answered by Dr. Sunil Kumar Srivastava (3 hours later)
Brief Answer:
bone infection often requires longer treatment

Detailed Answer:
hello
If marrow infection is persisting, your doctor would treat accordingly.
-re-imaging may be considered.
As informed earlier, marrow (bone) infection usually takes longer duration of treatment with antibiotics .

hi doctor .my leg is fine now. as i mentioned i have knee and foot oestoarthiritis too.i have put on little weight due to lack of exercise,my daughter insisted i do static cycle in a gym ,which the trainer made me do for 15 min and then walk on treadmill for 5 min ,the pain in both knees started , so m very worried weather to do cycling or not as my knees have improved since past 2 years after taking collaflax XXXXXXX and ccm tablets, but now i can walk leauserly for half an hour to like market and mall walking. plguide me wat exercise to protect my knee thanx
doctor
Answered by Dr. Sunil Kumar Srivastava (13 hours later)
Brief Answer:
cycling within pain free limits

Detailed Answer:
Thanks for new query-
Exercise based programs are started gradually and shorter duration to begin with.
In my opinion, if one has OA knee , then casual walking twice a day for 20 minutes and flexing /extending knee actively for 5 minutes before starting walk sitting on a table with legs freely dangling is good enough.one has to be pain free with exercise and limit timings accordingly.
cycling too within limits determined by the duration which brings it on
In any case, excessive wear and tear should be avoided by restricting strenuous activity.
The purpose is to 'keep the joints XXXXXXX periodically'

Pl help

Answered by Dr. Sunil Kumar Srivastava (22 hours later)
Brief Answer:
restrict movements,ankle support,anti-inflammatory

Detailed Answer:
Hi
Sorry for delay in reply..
I would suggest to not to speculate for the new problem-it may not be due to bone infection of right fibula. It would be good to show to an orthopedic doctor and stop worrying...
ankle pain without swelling into the joint may be some strain..i would like to restrict movements of the ankle for acute pain along with anti-inflammatory medicines and suggest hot fomentation, application of voveran gel locally for 2-3 days.it should settle...otherwise consult orthopedic doctor and have an x ray done if he suggests
Also wear an anklet or ankle support for few days with restriction of movements.

Hi doc my ESR 39 , rest all reports ok but MRI shows fluid in bone that is marrow edema, started feeling very sick in between so there's a orthopaedic friend who has put me on augmentin 1000 twice a day but he is still not sure weather it's infection or inflammation , pain is the same and he has told me to take rest , area is warm and little ref

This doctor says I cannot take augmentin and ceftum together , on the contrary last time fortis doc gave me this combination with which I recovered quickly , this doc has given me chinmoryl forte too
Have uploaded the reports too

Brief Answer:
evaluation for immunoglobins status, continue prescribed medicines

Detailed Answer:
hi
I believe follow up should be from where there has been some good outcome.
Repeated bone infections may require evaluation with an immunologist also. Some immunoglobin tests may be necessary,in case found deficient, can be remedies in the form of globulin infusions, for which an immunologist physician is a best judge.
As of now, the prescribed medicines may be pursued

Brief Answer:
ati-inflamatory medicines for tedinitis.Further evaluation for repeated inf

Detailed Answer:
please write your report details.Pdf uploaded by you do not open.
I have given my opinion for immunological consultation for repeated infections.
Tibialis posterior tendinitis, however, may not be due to infection and anti-inflammatory medicines, local supportive strapping may be considered and restricted movements during short course of anti-inflammatory medicines.
Your MRi suggests similar picture as of fibula.
Sometimes bone scans are done if MRI does not give clue to bone marrow infection.
Your previous doctor may be able to clarify the requirement.compare the previous MRI finding with this one.
