Can you please attach a picture of this wound? It would be much easier to answer this question? What kind of surgeries were done and when was the last one? Does he have any stool problems like blood in stool, mucus in stool, diarrhea?
Tunneled wound on butt cheek can be caused by ingrown hair (we call it pilonidal cyst) or some unnatural connection between the skin and rectum.
Pilonidal cyst can be seen as a small hole in the skin. As ingrown hair is the reason, tissues get irritated and create a kind of cyst (or tunnel). Such cyst gets infected from time to time so an abscess appears.
Unnatural connection between rectum and skin is called fistula. It can give exact than pilonidal cyst symptoms. As there is connection with large intestine, wound gets constantly infected. Fistula is more possible if he has any stool problems and can be a sign of inflammatory bowels disease.
Both lesions should be diagnosed with an ultrasound and fistulography (injecting contrast inside and performing an x-ray can tell where does this tunnel ends). MRI might be helpful in some cases.
Pilonidal cyst should be widely removed. Fistula should be closed, otherwise it wont get healed.
