Question: 70 year old husband started 100 mg Serequel 4 nights ago....having vivid dreams of a "third dimension"....Diagnosis: advanced frontal lobedementia....he has been in a very confused state for 8 months...not able to sleep....His thinking is clearer...able to hold a conversation...mouth movements and hand shaking 90% improved...now walking unassisted...is now continent....overall feeling better....However, he confuses the dreams with reality....

Firstly i would.like to tell you that he can get better with medicines.
The patient has complaint of frontal dementia- there are many personality and psychological changes associated with it.
The symptoms mentioned in.the query are related to the same. This symptom can be psychotic / delusional in nature ( but a more detailed questioning is required to diagnose it).
Had i been treating him, I would have given him resperidone 0.5-1mg or quetiapine 25-100mg at night.
These drugs will take care of the symptoms of dream and relality confusion.
After few weeks.of treatment, one.may be able to reduce his dosage to lower level.
It is advisable that patient should not move around after taking these medicines as he might fall and get injured [ the drugs produces some sedation]. The sedative part reduces with time
For sleep- he might need zolpidem 10mg for few days (in case when the resperidone or quetiapine.donot give sufficent sedation)..
With improvement in sleep - there is consederable improvement in overall symptoms.
