Suggest Treatment For Fungal Skin Infection
Let me give you brief history
1. During college days I had contracted tinea crusis in pubic area and lower testicles some 15 years ago. It was treated with ointment followed by biopsy to ensure it is gone.
Though hyper pigmentation remained which got lighten over years.
2. While taking steam bath in gym I had contracted tinea versicolor in small area (2 in in diameter) on my back and another fungal infection between the fingers web just on the tip. This was 5 years ago.
Both followed application of anti fungal lotions and oral intake of pills.
I stopped steam bath since then.
After these two i have been careful in hygiene, wear cotton, normally take bath twice a day, keep body dry and wash & dry linen separately.
Recently I observed two things:
1. Recurrence of fungal infection between of finger webs, on tips. I reported in HCM forum and started with Chlotrimazole lotion a week ago.
Still observing it to see if it gets better.
2. I also noticed dark pigmented skin between inner butts from cleft till lower of testicles. It is not visible in when standing up.
This is not itchy or scaly.
3. However on left inner butt area, there appears to secondary infection. Its continuous thin area running along from top to bottom.
It is hard, flat and dry. Becomes inflammatory and reddish on touch and tends to develop small cut like marks.
4. I also developed a small , scaly, reddish rash behind my left year. It does give an itching sensation on touch. I have been ignoring this for long time. Looks like it is also some sort of infection.
Life style wise i have a desk job and fairly active in sporting activities.
Condition on butts make me uncomfortable when sitting for long durations due to rubbing against skin or clothes or when indulging in sports due to sweat.
I am attaching photos both for ear and butts for better diagnosis.
Hope your advice shall make me better. For medication let me know exact brand names of medicines.

I understand your concern regarding your skin problem and I hope I am able to help you in the same.
I have seen the attached pictures.
You surely have an eczematised fungal infection over the groin folds, perianal region and the area behind the ear.
But I would like to know certain more details from you to find the reason for such recurrent infections.
Which all areas of body are involved?
Do you have any itching or scales over the scalp as well?
Do you apply any oil or moisturisers over body or the groin area on regular basis ?
Have you ever got your blood glucose levels checked?
Also please provide certain close up pictures showing the hand lesions also ,do that we can be sure that all the areas have similar problem .
How much is the associated itching.
I am sure I will be able to help you better once you provide me the pictures along with the above details.

To answer your questions.
1. No other body part is involved other than what I have mentioned.
2. I do not have any itching or rash on scalp.
3. I do not apply any oil or moisturiser on groins
4. I did undergo full body check up last year. Glucose levels were normal. Otherwise I am not obese, weight normal as per BMI. I have to go undergo checkup again sometime this year. So shall keep this mind.
5. Hand lesions are close to non existent now. I really cannot make it out now by naked eyes. They were just on the tip between finger webs. Though I shall continue Chlotriamzole for another 5 weeks on doctors suggestion.
6. There is no itching involved unless I touch these areas.

Thanks for all the details.
So basically you have multiple areas affected by fungal infection .
So it is important that you take oral treatment as well.
Apply candid b cream over all the affected areas twice a day for two weeks continuously.
In addition , take tablet fluconazole 150 mg once a week for three weeks.
You can continue to take levocetrizine tablet once a day for itching for as long as required.
After two weeks , you can start using flucos powder and dust it over the groin daily twice a day . Thus will prevent any recurrence of the infection.
Avoid application of any oils or moisturisers over body.
Wear cotton clothes as much as possible.

One question, the dark pigmented area and the hard flat rash on left inner butt that I described and can be seen in photo, are they same or are they different infections?

Hi and welcome again
The dark pigmented and scaly rash is the fungal infection .
The linear raised line in the peri anal region is basically normal .It is the normal slight hypertrophy of the skin which is seen in certain people. You really don't need to worry about it or do anything about it . It us normally painless .It tends to pain and develop cuts when the area is affected by fungal infection .
So the treatment that I have told will help in treating the fungal element .
Now another thing you need to know is that fungal infections over the body tend to heal with pigmentation which is what have happened in your case too .
Right now since the infection is active ,so I have given you antifungal for application . Once the fungal infection heals, then you can apply a lightening agent like kojivit cream to the dark areas twice a day to restore normal skin colour .

It has been 2 weeks, looks like infection is not responding to Chlotrimazole lotion.

I have seen the attached pictures .
There are two possibilities that I can think off.
Either it is mild fungal infection in the finger web spaces or it is simply a fine exfoliation of skin seen during change of weather called keratolysis exfoliativa.
So I would advise you to apply candid b cream over the area twice a day .
In addition, apply moisturisers over the hand two to three times daily .
It should get better with treatment.
