Suggest Treatment For Gender Dysphoria
Question: hello doctor,
I am XXXXXXX XXXXXXX i am 21 years old,
I am a freelance software consultant
and since i was 10 i am having a feeling that i am biologically male but i seriously believe inside i am a female (woman), i always hate to wear men dress even i am feeling really uncomfortable wearing men dress and more seriously i am not attracted to women, i am always attracted to woman dress and accessories. but more seriously i am really attracted to men i am not able to control my erection when i see men, even i am having little breast tenderness and pain in my nipples when i am horny, but my male genitals are functioning well, but when i want to dream about having sexual intercourse i can only visualize my self as a woman, when i see girls i am not attracted to their body but i am feeling jealous and i am a good software programmer but i am not able concentrate on my career because of this, i have feminine body, i have lot of fat in my chest area and i have wider hips so i have to wear free size dress to hide it, and i want to really grow my hair but when i cut my hair i am feeling really stressed and i really have feminine walk but i have to control my self to walk like a man, some times i have a feeling to cut my male genitals my self and i am feeling really uncomfortable to touch or be close with a man, and day by day these symptoms are growing as a big problem for me, even i have feminine face when i shave so i am not able to shave my facial hair and i love to grow my nails and i want to polish them with colors and i want to get rid of my body hair and i have lot more desires like a girl have and i read some online resources and watched videos and i decided that i may have gender identity disorder and i am not sure what i have to do next, i had some advice that i have go to a psychiatrist who can test me and provide me an approval that i can go for a hormone replacement therapy and sex reassignment surgery and i am not financially well for this but i can work hard to get assigned to a perfect woman body so please help me to live my life as i want and i am writing this email because i am really feeling uncomfortable to directly come so please reply me with your questions and tell me whether i should come directly and tell me how can i pay for consultation and i can get back to you with my answers via skype and i can pay you through bank neft transfer if you want and i have tried lot of my local psychiatrist and no one is supporting and you are my last hope please help me ,

You have a desire to get rid of the secondary sex characters and adopt those of the desired gender ie female so the diagnosis is very clear. This is an issue having huge psychological,social,financial ramifications so one has to think 100 times before deciding upon what is needed to do because once started, it is a irreversible process There is risk of side effects of medications, social taboos, and below expectation transformation , not compatible with sex life . I have seen some people managing married life and sex with females with intrapenile inj , penile prosthesis or even getting an erection by certain sensual fantasies , even with their wives . Yes, the guidelines say that to be examined by two psychiatrists to rule out psychiatric comorbidity. You can get your chromosomal analysis done from a good lab to see your genetic make up.

i am very clear about my symptoms and i have read lot of resources and contacted some transgender people to know the path of transition , i have already decided to go for the transition but i would like to start my hormone replacement therapy but can you provide me a diagnosis report by examining me through a internet call, i have tried lot of doctors here no is supporting so please help me by providing a report so i can start my new life....?
