Suggest Treatment For Grey Patch With Slight Redness Near Groin

default
Posted on Mon, 15 Dec 2014
Question: Hello Doctor,
I have a grey patch with slight redness near my groin area on both sides from many months with slight bumps (which appear/reappear/vanish at different spots). Itching is very rare and occasional. I have been applying betnovate plain cream after bath, it softens and makes feel better but it couldnt completely cure. From wikipedia, I could find that this is close to "Tinea cruris".
Could you please suggest some medicines?
Thanks in advance.
Regards,

Brief Answer:
Fungal Infection; Oral+topical antifungal

Detailed Answer:
Hello. Thanks for writing to us at healthcaremagic

I have gone through your query and I have understood it.

I think you most likely have a fungal infection/tinea crurus in that region. Betnovate contains a potent topical steroid, betamethasone valerate and is not recommended to be used for this condition.
This cream will only provide you symptomatic relief from itching, redness and scaling. That is because it contains steroid and steroids are antiinflammatory! You should stop it immediately.

You ideally need to use a topical antifungal along with Oral antifungal.

If I was the treating doctor I would have started you on oral antifungals e.g either fluconazole or terbinafine.
Oral antifungals are faster acting and control the infection much better when used alone with topical antifungal creams.

In addition to oral antifungals, I usually ask my patients to use a topical plain antifungal e.g either clotrimazole, which is available OTC Or one of the prescription antifungal e.g Sertaconazole, Luliconazole, Bifonazole etc. They need to be used regularly, twice daily for 4-6 weeks.

As itching is bothersome in fungal infections, therefore, I also ask my patients to take an OTC oral antihistamine e.g cetrizine 10 mg once daily.

Fungal infections can recur, if the conditions favorable for fungal proliferation start to exist again.
Therefore, an antifungal dusting powder e.g clotrimazole dusting powder is usually part of maintenance regime in my patients, once the condition has resolved with oral+topical antifungals.
Antifungal dusting powder keeps the folds dry as well as is inhibitory to fungal growth.

I would suggest that you visit a dermatologist in your region for a confirmatory diagnosis as well as appropriate prescription.
