Question: Hi. I had a concussion one year ago. Prior history of migraines, substantially diminished by time of accident to nearly zero with TCM. Accident recurred migraines more severe than prior and frequent. Previous diagnosis of Ankylosing Spondylitis, now in remission. Fast forward to now, current diagnosis of chronic Lyme disease and post-concussive syndrome. Demands on my time and energy have exceeded my capacity for one month, and last Friday constant headache started in pretty severe. Monday, body work on occiput (continued bodywork since accident last year). Monday, worst migraine absolutely of my life. Didn't know how I'd make it through night. Did Acupuncture on myself, slight improvement. 4x total self Acupuncture this week, Chiropractor tx Wednesday made slightly better. Heavy work week. Tonight again feeling like Monday, migraine simply awful! No confusion. No vomiting this sequence (although normally I do with migraines). Never ever had high blood pressure once, tonight with the pain it is 168/108. Urine is bright yellow (likely Vit B12 supplements recently). I manage stress well, although overworking taxes my body due to the chronic pain & fatigue I experience from underlying health condition. Post-concussion was layered on top of that. I don't feel worry & feel calm despite the excruciating, unbearable pain.

Hello dear,

I read your message carefully and understood your problem.

I see you migraines got worse after the accident. You have ankylosing spondylitis now in remission. Also, you are diagnosed with chronic Lyme disease and post-concussive syndrome.

Sudden headache which is more severe than the other times and associated with high blood pressure must not be neglected. This not a simple headache like in the migraine episodes. You need proper care and of course, go immediately to the emergency.

After the accident, a year ago some trauma to the head may have happened. It may have weakened some structures or have affected the blood vessels of the brain. Now that the process has progressed you feel the new symptom - headache associated with hypertension which means that this is due to the high intracranial pressure.

A hematoma, aneurysm, even Lyme disease itself can cause a headache of this kind. A head MRI will help to identify if the cause is because of Lyme disease who have affected the brain or because something else triggered after a year when you experienced the trauma.

Hypertension should be immediately treated adequately by medical team. So, I highly recommend to go to ER for immediate evaluation and examinations and consult neurologist as soon as possible to rule out brain damages.

You also mention to be taking herbal supplements, please check their ingredients as sometimes their use can also cause headache with increased blood pressure.

thank you. i didn't know if going to the ER was an over reaction. The herbal supplements I took should have decreased the blood pressure, therefore it seems important to go. Its hardest to know for oneself, always easier to recommend to others to take precaution. With the history of migraines, I've been through terrible pain before, but never an eight day migraine and never with increased blood pressure. Thank you for the advice!

Thanks Dr. Albana! IV drugs, hydration, and Oxygen therapy got pain down to 6/10 and blood pressure to normal. Getting to go home now. Thanks very much for your help!
