unfortunately there are no magical solutions...
When lying down you should elevate the scrotum, for gravity to assist drainage. There are scrotal support elastic fabrics that keep the scrotum in the best position and also apply a mild compression to help the fluid get drained but even an inflated (or filled with water) single use latex glove may do the job sometimes. The compression has to be pretty mild, to avoid blood flow problems.
If you have to walk, then a sling can be used to keep the scrotum in position.
More aggressive treatment of the decompensated CHF is also recommended. You should contact your cardiologist for individualization of your treatment, which would include increasing your furosemide dosage. Sometimes spironolactone helps (particularly patients with very low ejection fraction).
Another point to remember for the future, is that since you have a serious problem with fluid management, you'd better check your weight every day, in order to detect fluid accumulation as soon as possible. Early treatment means less discomfort.
