Suggest Treatment For Hemorrhagic Ovarian Cyst

default
Posted on Tue, 17 Nov 2015
Question: Hi. I am waiting to have a follow-up ultrasound next Friday for an ovarian cyst. Still very nervous, and I am constantly seeking out reassurance. Cyst was first noticed back in middle of August via an vaginal ultrasound(all I was told about that ultrasound was that everything was fine...cyst was not mentioned). Also had an abdominal ultrasound and everything was fine there, too. Come September 24, I had an a ct scan per my gastroenterologist because of stomach issues. There the cyst was seen (4.7x2.8 cm)...saying possible multiloculated. I went to my gyn a week later. At that time, she told me that a "baby" cyst was seen on each of my ovaries when I had the pelvic ultrasound in August. (My guess is it looked benign so no issue ). She says she believes the cyst is a hemorrhagic cyst...She has stated several times that she feels a very very small chance of malignancy so that is why she has had me wait until next Friday to have a follow-up to see if it is resolved. So my questions are how likely can you tell that a cyst is hemorrhagic when it is very small? What make those grow? Would it be normal for it to go from baby size to the size I indicated in 6 weeks? How fast does it take for cysts to dissolve/shrink? Can they just dissolve quickly? She has said to be if there was concern, she would have had me in there getting it out. I am a very anxious person, and this whole thing is just spinning me out of control. Any feedback, reassurance would be great. Thank you.

Brief Answer:
Most probably hemorrhagic cyst, needs further evaluation

Detailed Answer:
Hello and Welcome to Health Care Magic Forum. I have gone through your query and I think that most probably it is a hemorrhagic cyst only, still to be on the safer side get CA-125 levels done . further management will depend on levels of CA 125 and sonogram results. I understand your anxiety on this issue but further course will depend on the above test results . Regards Dr Manisha Jain .

Thank you. I think my gyn thought not doing the CA 125 because of the false positives and false negatives. She did not want to increase my anxiety in case it came back with elevated levels. So at this point I have to wait until next Friday. Can you provide some feedback to some of the initial questions I asked...for instance, can you tell the features of a hemorrhagic cyst on a small cyst? Is that a normal time to grow? What causes them to decrease and increase in size? How fast can they dissolve/Shrink? Thank you for providing answers to these questions.

Brief Answer:
Malignancy unlikely

Detailed Answer:
Hi dear, sorry for the delay in reply.
I know what you are feeling about the health issue and indeed anyone can get scared thinking about cancer seeing the rising incidence these days.
Most likely your gynecologist is right and it is an hemorrhagic cyst only with minimal chances of malignancy.
Yes the cyst can grow from that baby size to this size at times but most likely this is a new cyst formed after follicular rupture in this menstrual cycle and thus will regress in size eventually in a couple of weeks without any remains.
It is common and is seen in 3-5% of patients and thus can occur in a months time.
CA 125 will most likely cone normal and I had advised it to you so as to relieve your anxiety in a documented way as in case it is only mildly elevated or comes to be normal then malignancy is largely ruled out.
Chances of malignancy are meagre.
I hope it was reassuring but still in case you have any questions about it, I will be glad to answer.

Thanks for your response . I understand the suggestion to have the 125 done ; I just know with chance of false positives it would send me over the edge. My gyn seems to think it was the same cyst that was first seen on the ultrasound . How accurate is a Ct scan with identifying cysts . They described it as possible multiliculated one. Would a hemogrraphic one possibly look like that? Also, do you bleed if it sheds ? Does it give you any symptoms ? Would I know ? And is there is anything I can take to help dissolve it? I read about some natural things but not sure if anything has any impact . Thanks.

Brief Answer:
Benign cyst

Detailed Answer:
Yes dear CT scan is very much sensitive and accurate so don't have any doubt on it.
Yes the multiloculated picture is commonly seen in a hemorrhagic cyst but is also seen in a number of benign disorders of the ovary.
CA 125 if raised mildly then also it is seen in benign cysts and diseases of the ovary so saying this mild elevation as false positive is not a rational thought.
It will not have much of a symptom while disappearing and will not give you much of the symptoms so you will not get to know about it much.
There is nothing much you can do to dissolve it in faster mode except for taking antioxidants daily in the form of fresh vegetables and fruits.
I know you are very anxious about it but there is nothing to worry about and most likely it is just a benign cyst detected accidentally.
Take care.
