I have gone through your query and pictures. It looks like hemorrhoids only.
You can do sits baths, take stool softeners like lactulose at bed time, take dried prunes and apply hemorrhoid creams containing hydrocortisone and anaesthetic like proctosedyl.
If not improving then surgical management will be needed. In such case a consultation with general surgeon or proctologist is well appreciated.

You can use witch XXXXXXX But the effectiveness is not promising.
Sits bath and hemorrhoid creams which i mentioned as well as stool softeners will be helpful to a great extent.
