Suggest Treatment For High TSH Levels And Leg Pain During Pregnancy

Question: My wife is pregnant and is on 4th month end. She is a bit obese. Her weight is 74.9KG and height is 5ft2inc.
1. Will the weight be a concern during baby delivery? Also should there be any change in medication she is following from the current doctor.
2. She is having high TSH. Will these be of any problem while taking the thyroid control medications on the baby?
3. My wife is also having pain in the tailbone and under the leg/ankle. How to get relief of the pain. Is the pain any alarming or normal.
4. As per 14Week USG, the baby weight was 0.96gms. Is the weight OK.
5. Lastly can you suggest any specialist gynecologist doctor who would also be available during the delivery of the baby. We are staying at XXXXXXX kadamtala area of greater XXXXXXX West Bengal.

Hello Dear,

Thanks for attaching all reports. I have reviewed them.

1) At 5'2" height, standard weight is 55 kg (max). Her Body Mass Index is 30 and it indicates Obese Class 1.

There will be increased risk of high blood pressure (Pre-eclampsia or eclampsia), Gestational Diabetes, premature birth, heavy bleeding after delivery of baby (PPH), deep vein thrombosis etc. Therefore, it needs to be concerned.

Proper care should be taken by the obstetrician during delivery. Mode of delivery would be C-section in this case.

2) Medicines will be continued as per doctor's prescription. Do periodic check up to her doctor. Check for blood pressure and blood sugar.

Take healthy balanced diet, more fruits and green vegetables, do light exercise or walking short distance for 1/2 hr regularly. These things will be beneficial to control overall weight gain.

3) At present, serum TSH level is quite NORMAL and prescribed medicine will be continued throughout the pregnancy period. There will be NO bad effect on the growing baby.

4) Pain around tailbone is quite common during pregnancy due to relaxation of pelvic ligaments as a result of Relaxin hormone. Foot or ankle pain is due to venous congestion and fluid retention. Following things would be helpful:

a) Rest as much as possible and keep your legs in high position than pelvis. You can use 2-3 pillows for the same. Reduce salt intake in daily diet, avoid sugary or fatty foods.

b) Do some simple foot exercise like bending and stretching your foot up and down for 20- 30 times. Use only flat heel shoe and avoid uneven surface.

c) Massage the area with lavender or camomile oil to reduce discomfort. Use ice pack or hot pad/ water bag compression around back region or foot. She can keep her feet in bowl of slight warm water with adding essential oils.

d) Don't cross your legs while sitting and don't stand or sit for long time in one place. Keep bowel habit regular. Paracetamol can be given as mild pain- killer. Maternity support belt will be helpful if tailbone pain is continuous or moderate.

5) Baby weight is quite NORMAL at 14 weeks + gestational age. You have nothing to worry about this.

6) You should contact with local gynecologist or private hospital in this case. Get an appointment and do periodic check up to him/her, who will also be available during delivery.
