Suggest Treatment For High Grade Stenosis In Carotids

Posted on Fri, 8 Jul 2016
Question: Hi Dr this question is for my father in law he had a stroke recently and is paralysed on the left side. He was treated at Cmc vellore and now continuing medication and physiotherapy at home. His doppler report states that it is suggestive of high grade distal ica occlusion. Yet the Dr chose to put him on acitorm 2mg for a week and take a Anticoagulant test after a week. The Dr didn't do MRI or CT angiography. My question is this correct treatment or he needs further intervention like surgery?

Brief Answer:
Get consultation from stroke specialist or intervention radiologist

Detailed Answer:
Hello,
I have gone through your question and understand your concerns.
This seems to be stroke due to diabetes, hypertension and carotid plaque.
What is the Mri brain and angiography reports.
High grade stenosis in carotids need intervention to avoid recurrent stroke.
Stenting and endartectomy both options are available according to patient status.
Get opinion from stroke specialist or intervention radiologist for further course of action.
Continue rest medications as such and physiotherapy too.
Hope you found the answer helpful.
