Suggest Treatment For Hypertension, Bloating, Gas And H-pylori Infection
Question: Hello Dr I'm a 35-year-old male I have been diabetic for the last five years and have been taking medicines for hypertension since 2004. My doctor prescribed me Domizol DS in 2005 for bloating and trapped gas in 2005 this medicine contained omniprazole and domperidone. In 2010 my doctor change my medicine to pantop dsr which had pantoprazole and domperidone. In 2012 i had a severe attack of bloating, inablity to pass gas or burp and i was hospiatlized for a day. They also coducted a upper endoscopy and confirmed H Pylori infection. I was given an HP kit for 14 days however since the symptoms aggravated i had to stop the HP kit in 5 days. Since then i was taking a sompraz L ( esomeprazole and levosulpride) in morning and sompraz 40 in night with pankreoflat after meals. I was symptomatically much better in 2013 -14. However for the past few weeks iam again experiencing severe bloating and inablity to burp. This time i do not have a major issue with passing gas however after eating anyting or even drinking water i feel a severe urge to burp and my abdomen bloats. While sleeping i get up suddenly with gas trapped between my stomach and throat. After a few minutes i burp and feel better but this happens several times and because of which i am not able to get proper sleep. In the morning also after breakfast i feel gas trapped in my stomach and unable to burp. My gastroenterologist has prescribed Gaviscon syrup but has not been very effective. These episodes have made me very anxious and i feel depressed. Please advise.

1. T.Lesuride 25 three times a day before breakfast everyday (till you have symptoms)
2. T.Rifagut 400 mg thrice a day for 2 weeks
3. C.VSL #3 once a day for 4 weeks
4. Effective control of blood glucose. Keep HbA1C level below 7.

I do have reflux sometimes but it is absolutely non acidic as i do not feel any heartburns. As you have pointed out that i may have gastroparesis which is a more severe disorder (partial paralysis of stomach) and is one of the complications of diabetes, however i have suffered from such condition even long back when i was not a diabetic. Additionally i usually do not feel that i have undigested food in my stomach during the day although if i do not get enough sleep during the night i do feel morning nausea with some refluxes but without any vomits.
Most importantly i have very regular bowel movements with proper stool as i get up in the morning and usually in the evening also. Is this possible in gastroparesis where there is a delayed gastric emptying?
How do we confirm if iam having gastroparesis could it be a less severe motility problem?
My current medication:
1. Sompraz L (esomeprazole 40 mg and levosulpride75mg) once before break fast.
2. Sompraz 40 mg before dinner
3. Syrup Gaviscon after all meals
