After reading short history and seeing photo,I could make out that most probable diagnosis is tinea corporis. There is an erythematous papulo macular lesions in annular pattern with central clear areas on lower leg. The rash spreads peripherally. There may be mild to severe itching. The lesions might exacerbated in humid warm weather. It is a fungal infection caused by dermatophytes. It might spread to other areas like upper extremities,chest,abdomen,back,groin and buttock. You consult dermatologist for firm diagnosis. KOH preparation for fungus might confirm the diagnosis.
The relapses of disease is frequent in this type of dermatosis. Blood tests may be done to rule out diabetes mellitus and HIV. Other immunocompromised conditions may be kept in mind. Poor hygiene and poor immunity might be responsible for the relapses.
I would recommend as follow..
- antihistaminics like levocetirizine once a day till itch is relieved
- antifungal like itroconazole cap 100 mg twice a day for two weeks
- to apply antifungal cream like clotrimazole cream
- avoidance soap bath..diluted dettol in water may be cosidered for bath
- improve immunity
- improve hygiene
- treatment of underlying diseases
