I have swollen veins, itching and bleeding in the crotch and inner thigh area.

Intertrigo refers to inflammation that occurs in the body folds. It can occur in the groin as in your case, in the axilla, between the buttocks

It occurs as a result of friction: (1) the skin folds rubbing together
(2)moisture
(3)heat

This causes the inflammation then bacterial and/or fungal infection can step in and make it worse.

Treatment involves addressing the aggravating factors, friction, moisture and heat.
(1)using absorbent powders if available eg talcum powder. Since fungal infections is likely with the history of itching, you may want to try zeasorb with has the talcum along with an anti fungal powder.

(2)separating folds/affected areas whenever possible at home and exposing them to air
Can use cotton or linen cloth to separate

Avoid any occlusive dressings to the area

(3) Consider a mild steroid to the area to reduce inflammation eg hydrocortisone, this would help to reduce itching faster

(4)consider barrier creams or powders as well for skin protection eg triple paste or XXXXXXX goo
Triple paste contains zinc oxide and petrolatum (both act as barriers) along with aluminum (helps with absorbing) XXXXXXX goo has nystatin (anti fungal), zinc oxide (barrier) and hydrocortisone (mild steroid

Use a lot at a time of either of these

(5)consider topical antibiotics as well eg neosporin, fucidinn

In some cases of severe fungal or bacterial infection where there is no response then oral meds may be needed

PLAN:(A) keep folds separate with the cotton or linen, exposure to air when possible at home, avoiding occlusive dressings


(B) if using the triple paste or similar product , add a mild steroid cream such as the hydrocortisone and also add the zeasorb

Use the steroid cream first then the triple paste then the zeasorb. Use the steroid cream for 5 to 7 days

Anti bacterial cream also used before powder

(C)If using the XXXXXXX goo which already has a steroid and anti fungal do not need to apply separately
Apply the powder after XXXXXXX good and anti bacterial cream

It can sometimes take a few weeks for improvement but if you not comfortable with the rate of change, you should see your doctor for assessment
