Suggest Treatment For Kidney Cyst

Question: My mother has a kidney cyst of 1 cm plus she is having a CRTD Device. Will she need to go for surgery????

I read your query and understood your concerns.

A renal cyst around 1 cm rarely require surgery. Surgery is required in case of pain and hematuria.

If you were my patient I would recommend to postpone surgery for the moment and to regularly follow up the renal cyst. If the kidney cyst enlarge further (more tha 1.4 cm) than you should go for surgery.

Is amiodarone advisabe medication for patient with a device system CRT-D to use for long period of life???? After surgery .

Amiodarone is advisable to be used for the long treatment of this patients.

Amiodarone is advisable to be used for the long treatment (over a long period) even in patient with a device system CRT-D. You can safely use it after surgery.

Since it's a medical therapy you should discus it with her doctor to choose the right therapy and the right dose.

Hi back,

i read very carefully the data you provided and I can confirm that she has a severe heart failure with very low EF (15-20%), but on the list of medication you are taking I can't find Amiodarone. for the moment you are taking carvedilol which is a betablocker.

Amiodarone is recommended to be used in such a patients only in case of ventricular tachyarrhythmia’s (ventricular tachycardia, multifocal PVCs) in order to prevent from sudden cardiac death.

Patients with low ejection fraction are complicated with life threatening arrhythmias and Amiodarone use is of help in this case. You should discuss with your doctor why he wants to stop amodarone. Amiodarone is stopped only if your doctor confirms a long QT interval on ECG, which is a side effect of all antiarrhythmic drugs (including amiodarone).

Hi back,

Your mother cardiologist after CRT-D procedure has added Amiodarone to her therapy in order toprevent her from life threatening arrhythmias and sudden death. Her doctor can do this now safely because CRT-D has inserted pacemakers in her heart and prevent from bradycardias and syncope, which are to major complications when carvedilol is used together with amiodarone.

If you want to avoid using for long term amiodarone than ICD (implantable cardioverter defibrillator) should be inserted on her to.
