Question: Sir, my wife (31 yrs) has been suffering low grade fever (99-100.5) for a month now accompanied by cough for almost 15 days now. In the first week, she did complain of abdominal pain and was also feeling very cold (chills). Earlier all her tests were normal. However after almost 10 days, her blood tests first showed a positive result for malarial infection (both P-Vivax & P-falciparum were positive). Then 3 days later, after completing the course for anti-malarial medicines, the blood tests showed negative on malaria, but now a weak positive for Typhoid. Her prescriptions have been as follows - Taximo 200mg (twice a day for 6 days), then the anti-malarial medicines for 3 days, then followed by 7 days of ceftum 500mg which has now been followed up by Moncef IV (2mg) twice a day for 3 days. Even after completing all the courses, she still has the same fever and cough. Its also worth mentioning that we had also done tests for TB (quantiferon gold test and moneoux test), the results of which were positive to TB infection in both cases. However her chest X-ray was normal. So the doctors haven't confirmed on TB as yet. She hasn't lost her appetite at all. Also her BP has been around normal and weight has been absolutely stable at 51 kgs. Another point to note is that her fever does come down very quickly after a dose of Paracetamol 650mg. Need your help in diagnosing what this could be ?
