Suggest Treatment For Low Stomach Cramps And Difficult Bowel Movement

Question: I have very low stomach cramps, feeling as if I h
have to have a bowel movement, but I cannot go. it has been 12 days. It feels as if something is stopping it at my rectum. I use dulcolax, but no help. I drink lots of water. It fells the more I strain the more tighter my rectum gets. It feels as if something has fallen out when I strain.



what should I do. Several months ago I tried to have a colonoscopy, but The laxative did not work. No one scheduled another one.

I read your question and i understand your concern. All your issues are due to constipation . Not able to pass stools, not responding to laxative of dulcolax has cause this straining and abdominal cramps and pain. Colonoscopy was done to rule out any organic cause like anal fissure, malignancy , obstructed bowel etc. Had there been the need for another one you would have been scheduled for.

Pelvic muscles physiotherapy may help as some times it is Inability to relax the pelvic muscles to allow for a bowel movement (anismus) or Pelvic muscles don't coordinate relaxation and contraction correctly (dyssynergia) or there may be Weakened pelvic muscles.

Anorectal manometry and colon transit study should be performed to see what is going on. An xray of the rectum during defecation ( defecography) should also be taken.

Drink more fluids, exercise regularly and don't resist the urge to defecate. Training the bowels is necessary. Early morning breakfast followed by 10 minute break and then sitting on the toilet for ten minutes is the best habit.

If dulcolax is not working you can change the medication to other types like stimulants or osmotic diuretics like ilk of magnesia, magnesium citrate, lactulose, polyethylene glycol (MiraLax) and sodium phosphate enema (Fleet Enema). Lubricants like mineral oil and stool softeners like Colace and Surfak can also be tried. Lubiprostone is also a promising drug.
