Suggest Treatment For Lower Abdominal Pain

Question: My daughter who is 7+years old complained of lower abdominal pain (which goes awaywith the passage of stool or gas) last month with fever (100 degrees) in her urine test she got 5-7 rbc all other findings were normal. she was put on antibiotic and i got her USG done. it was normal. thereafter her KFT and 24 hrs calcium creatinine was also done. both test were also normal except with low calcium levels. she recovered in 2 days and is well since then. However after stopping antibiotic (after 5 days) within a gap of 2 days urine routine was done again. It showed the same RBCs i.e 5-7. after three weeks, it came down to 2-3 rbcs...Shall i investigate her further

I have a few questions for you before I answer your queries -

1. Does anybody in the family is having a history of kidney stones?
2. Does she have any history of bleeding during urination?
3. How many pus cells were there in the both the urine tests you have quoted? I am asking this because RBC are not related to infection and hence I am in doubt why at all antibiotic was used when there is no infection.
4. Was any urine culture done before starting antibiotics?
5. What are the current weight and height and exact age of the kid?

As of now I can only say that - NO FURTHER INVESTIGATIONS ARE REQUIRED.

Answers to your questions:-
1. No history of kidney stones but just 3 months ago the daughter of my brother diagnosed with a stone which has gone now. this is the only case in my entire family of 4 brothers and 3 sisters. No history from wife's side

2. Never noticed bleeding. Frankly saying, we never tried as there seems to be no need. Urine test done only when she got ill and then RBCs come.

3. Pus cells 2-3 both the times
4. I am also surprised why antibiotic prescribed as the urine test was done before starting of antibiotic. Culture report came after 3 days which again did not show any infection

5. Her current weight is 22.5-23 kgs...height almost 4 feets and age is 7 years 8 months.

Kindly advice what it is and what the further course of action. Let me tell you one thing Sir...i am a person of acute worrysome nature so asking questions from all sources and getting more worried.

1. This is not urinary infection. So no need for antibiotics.

2. This is not kidney stone either.

3. But your doctor was right in evaluating for calcium and creatinine in the urine and he has rightly ruled kidney stone int he kid. So do not worry about that too.

4. YOU NEED NOT INVESTIGATE FURTHER.

5. This could be an inter current mild viral illness and she has recovered form it. Do not worry.

Thank you Sir for replying.

I have been getting a lot of different advises so getting confused. since you have suggested that no further testing is required, what should I monitor in her in terms of physical symptoms.

I do want to get her treated and cured completely in a best possible way but at the same time do not want her to run from pillar to post in Hospitals.

Hi....these are the things to monitor in your kid -

1. Red or cola colored urine.
2. Pain during urination.
3. High grade fever.
4. Continuous vomiting.
5. Pain in the tummy which is intolerable.
6. Loss of weight.
7. If urine is cola colored then the kid's blood pressure needs to be checked.
