I read your question and i understand your concern. Your question has basically two components. Lump in the kidney and constipation likely attributed to the back pain and abdominal pain.

As far as the lump on the back is concerned that needs examination, assessment for the nature and type of the lump and also for the extent of involvement and labelling it for the origin. All that needs a combination of naked eye examination, complete clinical correlation and a detailed thorough work up which is beyond the scope of this forum so you would need to see a general surgeon or any other doctor PCP to have a look at the lump and decide what it is.

As far as constipation is concerned , we all know it can not be treated, just be managed. And there is no better way than modifying diet and lifestyle. Fibrous diet 14 gram of fibers for every thousand calories, minimum of 8 glasses of water in a single day, pelvic floor muscles training and if need be surgical intervention like sacral nerve stimulations etc.

Apart from it as per need basis use of laxatives will help. Prefering vegetables and avoiding meat or any other proteinaceous low fiber diet would be beneficial.

For the pain i would suggest restrict to acetaminophen with muscle relaxants like skelaxin as opioid will worsen the bowel movements and NSAIDs will make the bowel habits compromised by causing unnecessary GI ulcers and inhibiting prostaglandins of the bowel mucosa.

Nutshell, get the lump examined and manage the pain conservatively and treat the constipation with mother nature.
