Suggest Treatment For Lump On Testicle

Question: My husband has been experiencing clear liquid leaking from a small white lump on his testicles. When knocked or rubbed it leaks enough fluid to leak through underpants and his trousers within a couple of hours. There is no discomfort though. He is 49 years old suffers from cellulitis of the scrotum and colitis. Should we be worried by these symptoms. What does it mean?

I understand your query and concern.

The leakage from the scrotum could possibly might have been resulted from fistula and abscess formation secondary to underlying cellulitis.

Its extremely important that your husband needs an urgent care to prevent gangrene infection of the testis.

Symptoms in his case are a matter of concern.

Uncontrolled infection can lead to destruction of testicles leading to sepsis.

I advise you to get him ultrasound scrotal Doppler along culture sensitivity of the leaking fluid to determine appropriate course of management with antibiotic selection.

I suggest you to consult a Urologist at the earliest for expert management.

My husband has been on anti biotics for 12 years now for the cellulitis. He is taking 250 mg of amoxycillen. He tells me that he has had this lump for two years now with it flaring up periodically usually when he knocks it. Does this knowledge make a difference to your diagnoses.

I understand your query and concern.

Since he seems to have this infection/cellulitis since 12 years,and the onset of lump like feel since 2 years, there could be two possibilities.

1.It could be a walled off abscess with recent fistualization either due to inappropriate antibiotic therapy or progression of resistant infection.

2.Suspicious new testicular growth.

A malignant growth has to be the prime consideration in any testicular swellings in individuals above 40 years of age.

Hence its mandate to have an Ultrasound Scrotal Doppler(with a urological consultancy), to clearly demarcate and distinguish the two as well as to plan further management,since the treatment aspects of the two are primarily distinct.
