1. It is probable that there is some fluid collection or a similar condition in the epididymis which is a structure found in the scrotum and separately from the testis. This can be a spermatocele and you are probably right. Other conditions can include a cyst in the epididymis or a small hydrocele.
2. A confirmed diagnosis requires an ultrasound scan and this will help to see clearly the area which is causing the lump. Therefore it looks like a benign cystic swelling to me.
3. Cancer is less likely is it is a small lump and been there for 5 years. Please get an ultrasound scan of scrotum next week and things will be confirmed.
