Brief Answer:
Infection of the abdomen is most likey.

Detailed Answer:
Hello,
Thaks for the query.

His symptoms refer mostly to an acute gastroenteritis (infection of bowels). For the time being, I'd suggest him to drink more water, take Tasectan, probiotics, and Dicyclomine for spasmodic pain.
The epididymal cyst may not be related to this.

Investigations suggested are-
Ultra sound abdomen
Stool test routine and microscopical analysis

Treatment suggested-
Tab Ofloxacin 200 mg twice daily
Tab Metronidazole 400 mg thrice daily
Tab Dicyclomine 10 mg thrice daily
Tab Zofran 4 mg twice daily

You may need a prescription from your local physician for these.

Let me know if I can assist you further.

Thanks!
