Brief Answer:
want to ask about primary site.

Detailed Answer:
Hello dear, thanks for your question on HCM.
I want to ask you few questions.
1. Where is the primary site of malignancy?
2. Which investigation you had gone through like CT or MRI or PET scan.?
3. Have you confirmed about the lymphnode of being metastatic only?
Usual treatment of aortocaval lymphnode is chemotherapy only.
Radiotherapy is contraindicated as chances of damage to the vital organs like aorta, heart and lungs are high.
So chemotherapy is the only option.
Hope I have solved your query. I will be happy to help you further. Wish you good health. Thanks.

Brief Answer:
it is metastases only.

Detailed Answer:
Hello dear, thanks for your reply.
Colon cancers are commonly spread in liver and thoracic lymphnodes. So chances of metastases is more in your case. And PET is confirming the same.
So it is clearly a drug failure. Or rather we can say it is the nature of tumour. It is in curable.
Metastases are going to happen even after successful surgery and chemotherapy.
Better to consult with your oncophysician and start the treatment de novo.

Brief Answer:
yes, it is stage 4 tumour and inoperable.

Detailed Answer:
Hello dear, thanks for your query again.
In my opinion if metastases seen in liver and thoracic lymphnode than it is considered as stage 4 disease and becomes inoperable.
But in recent era single isolated liver metastasis can be operated and removed.
And aortocaval node can be treated with chemotherapy.
So I advice you to consult oncophysician and discuss about these options.
If patient is fit(medically) for surgery than selective resection of liver metastasis can be an option.
So better to discuss these in detail with your doctor.
