So subpleural lobe is not an organ it is mostly accessory lobe.
Metastatic nodule suggests you are having primary cancer somewhere else. If this is the case than your primary cancer is staged as stage 4 disease and it becomes inoperable, only palliative care should be done in the dorm of chemotherapy and radiotherapy.

January scan showed a nodule in the sub pleural lobe .
How come the radiologist did not state the measurements of the nodule ?

It is quite possible to have resistance to chemotherapy drugs.
So radiotherapy can be the option in such cases.
But again risk-benefit ration after giving radiotherapy should be discussed in detail with Radiotherapist.
And surgery of metastasis (metastesectomy) can be done. But the risk of surgery and post operative complications are very high. So again you need to discuss risk-benefit ratio in detail with oncosurgeon.

Can the three mets b ablated ? There's 1 on sub pleural, 1 in liver and 1 aortocaval node. All small .
Plz can u finally also advise re survival length ? Can he still have a few years or not

There is targeted beam radiotherapy available. This targets the metastatic nodule and destroy it.
It is specifically given in isolated metastasis.
Radiotherapist mostly prefer cobalt for this.
So better to consult radiotherapist or oncologist for this therapy.
And if his primary tumour is under control than these small metastases should not be operated because operation is itself very challenging and outcomes are poor. Better to go for radiotherapy.
And prognosis is good if he is having only these 3 small mets. Five year survival rate will be 50% and life expectancy is almost 2-3 years.
So he is still having years to live, so consult oncologist and start appropriate treatment.

Systemic chemotherapy is the only option to prevent micro metastases in the body.
So better to consult oncologist and start chemotherapy other than xeloda and irinotecan.
