Question: I recently started running again after a 15 year hiatus and have begun to have pain just above foot in front of ankle. Any way to guess if a stress fracture or very low shin splint?
There is mild swelling and redness.
Age 66, male. Was jogging/walking 50 miles/wk for about a month prior. Mostly avoid sidewalks.

Brief Answer:
As mentioned below

Detailed Answer:
Hello dear XXXXXXX castiglia, Welcome to HCM,
Understand your concern,

Your symptoms could be a synovitis, extensor tendinitis, splint and/ stress fracture.
Pain increased on extension of foot suggests extensor tendinitis.
Pain when you push on your shin suggests splints.
Fracture and synovitis should be assessed by foot X-Ray and physical examination.

Take following advises in the meanwhile:

Wear Ankle support splint.
Do not stand for long time or jog for next 2-3 weeks.
Take Anti inflammatory and analgesic(eg. IBUPROFEN 400 mg Thrice daily after food for next 3 days)

Soak your leg in warm water with Regular/Epsom salt.
Wear proper supportive shoes during jogging.
Continue to apply ice pad, heating pad massage.

Hope I've answered your question, let me know if there's any further queries
