I can understand your concern. The muscular spasm, radiating cold sensation and pain in both legs along with twitching in arms and buttocks could be due to
: Upper motor neurone lesion like spinal cord compression in neck. It can be due to disc
mass lesion
post traumatic
It can be suspected more if you are having tight muscles, increased tone, focal deficit, some neck pain also, spontaneous appearance of symptoms which are progressive.
For confirmation, consult doctor. He will advise MRI cervical spine to rule it out. Further treatment will depend upon reports.
: Deficiency disease - There are so many deficiency diseases which can cause such symptoms like potassium, calcium, dehydration, vitamin like B12 (which you are already taking), folic acid, E.
Deficiency also causes mouth ulcer, fatiguability, weak bone, burning feet
It can be confirmed by checking blood level of minerals and vitamin. Treatment is
Increase fluid intake
Increase intake of food which contains potassium and calcium like banana, dairy products, sour fruits like lemon/orange/pineapple, eggs, meat.
You can take multivitamin supplements containing B12, folic acid, calcium and vitamin E.
: Chronic fatigue or exertion - It can be work related, sports related, shifting to new place, home related. If you are doing severe extortion for few days, it can cause all such symptoms.
Treatment is simple
Bed rest for few days
Increase fluid intake of water, juices, saline-lemon-sugar water
Full body massage
: Polyneuropathy or myopathy - It means multiple peripheral nerve or muscles involvement. The reason could be deficiency disease (B12, folic acid, calcium, protein), diabetic, alcohol, smoking, familial, toxins ike paint/chemical exposure at work/pesticides, drug related like Metronidazole/chemotherpy, familial.
It can be confirmed by complete history and examination of nerve-muscular system,
Nerve conduction velocity of peripheral nerves
Electromyography of muscles
MRI of spinal cord and suspected nerves
Blood investigation to rule out deficiency/diabetes
If still doubt, biopsy of nerve or muscle.
Treatment depends upon the cause. Usually management is medical.
