Suggest Treatment For Nodulocystic Acne
Question: Acne of face not getting cured. I changed my dermatologist and the new doctor says I have Acne Nodulocystic and would take about10 months to cure. He prescribed Sotrat 20 mg for 30 days daily. My questions are:
- Is this right diagnosis
- Is it advisable to consume alchohol (Consumption is about 80 ml a day, twice a week) while I take Sotrat. If no how many hours gap is required between medication and the drink.
- As per my lipid profile I have triglyceride levels exceeding 300, is this linked to my present diagnosis.

The drug which you have been prescribed is an Oral Retinoid i.e Isotretinoin.
Nodulocystic acne is the severest grade of Acne and Oral Retinoids i.e Isotretinoin is the drug of choice for severe Nodulocystic Acne.
Retinoids are also indicated in those in whom Acne is not responding adequately to traditional antiacne medications i.e Topical antiacne preparations and Oral Antibiotics.
I can see from your treatment history that oral antibiotics were not able to control the severity of your acne.
Alcohol, as such has no direct interaction with Isotretinoin.
However intake of alcohol might be one reason for raised triglyceride levels and it would be better if you can stop it.
Raised Triglyceride level is a concern; baseline testing is usually performed before starting Oral Retinoids. Normal levels are usually less than 150 mg/dl. Your levels are almost double the normal levels.
Serum raised triglycerides is not the reason behind severity of your acne.
However, In those who have a high triglyceride levels may not be the right candidate for Oral retinoids because Retinoids might raise the levels even further thereby increasing cardiovascular risks.

Thank you for the detailed answer. It concerns me and I am therefore not going to take the Oral Retinoids prescribed due to fear of further aggravating mytriglyceride levels.
