I have gone very carefully through your query. Your problem seems to be due to gastritis or gastroesophageal reflux. Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. If somehow this movement gets hampered, food along with the digestive juices would stagnate, stomach acid will reflux into the food pipe and on the other hand the bowel habits would not be proper.
In order to relieve yourself, you need to do the following:
* Avoid spicy food. Take small but frequent meals. Avoid long gaps between meals.
* Avoid alcohol intake (if it a part of your habit).
* Take Prilosec (omeprazole) once daily before breakfast.
* Take plenty of green leafy vegetables as well as other sources of dietary fibers. Take fiber supplements like isabgol/psyllium husk or senna. I would insist you to increase your dietary fibre intake.
* Regular morning walk and exercise to keep the bowel movements proper.
* If I had a patient like you I would perhaps have suggested Prodep 20 mg tablet once daily for at least 2 months (it may take time for the action to set in).
* Take curd or yogurt on daily basis. It contains beneficial bacteria and should help.

1) Does it harmless to take daily Prilosec (omeprazole) or anti-acid tablet for longer period?
2) Also one tablet of Prilosec (omeprazole) or Omez tablet in the morning does not help; she required to take another tablet in the evening; does that is appropriate? if not than what will be the solution?
3) She is also complaining throat irritation & hiccups; does those are also related to same problem or we have to consult ENT specialist?
4) She is having the higher obesity issue; does that is required to be treated to reduce this problem? (Her height is 5.3 feet, Weight is 81 KG)
5) One of the Doctor, suggested to treat this with Surgery; does that is necessity or if we follow your suggestions we can control this problem?

1. It is overall safe.
2. Twice a day can be taken. But if the reason for it needs to be fathomed. Use of Prodep needs to be considered for the same.
3. The hiccups are likely to be related to the same gastroesophageal reflux that is the cause of her taking Omez. ENT consultation would not help.
4. The obesity is indirectly related to the issue and reduction of obesity will help in taking care of this problem.
5. The mentioned surgery is to reduce the size of the stomach. If she does not have control over how much to take in, this is the option to consider. If she can self monitor and restrict the amount of food intake, that is perhaps not essential. Not everyone has the same level of self-control and discipline.

In place of Omez can she take Famotidine Tablets; that is Famoacid 40?
