Hai Mam
Thank you for asking to healthcaremagic..
Seems like he needs ER ASAP to prevent embolism possibly to lung vessels..In ER.they will check the patency of the blood vessels, which is the first and potential cause to be ruled out..
Others can be dealt later
1)Is the pain and swelling intermittent or permament
2)Was he had any previous such history ? i.e new onset ?
3)Any visible veins or bulging veins over the leg ?
4) Is he ambulatory or bedridden type of individual or obese ?
Please provide me this relevant information to further assist..

Brief Answer:
Needs muscle relaxant
Detailed Answer:
Hello there..
In case of severe charlie's horse, a combination of treatment modalities like stretches under guidance of physiotherapist, pain killers (like ibuprofen, aceclofenac) and muscle relaxant (like cyclobenzaprine, etc) are advised. A consultation with orthopedic surgeon is advised.
I didnt understand your last sentence.
On what basis you are saying he is bipolar? Lithium most probably would have caused acute damage if taken without proper supervision..Hence bipolar disorder first of all is very unlikely...
Let me know if I can assist you further.
