Suggest Treatment For Pain In Ears And Headaches

Question: Hi I am a 44 yr old female, and I have had an slight earache on my left ear, crunching noise in my head when I make movements side to side, headaches but mainly on the left side and sometimes dizziness when I lie down and would turn on my right side. I have suffered with this since January 2015 of which I was given a few diagnosis which is, ear infection, vertigo and deep sinus. The vertigo has been treated with the lying on a doctor's coach and turning of the head was performed, but apart from that, the other symtons still exist of which I now get pain in my right ear or both ear. It's not extremely painful but I always feel pressure in my head. I was advised to ask my GP to refer me to a specialist but so far I have just been given antibiotics which did not work and only made me constipated....

I have gone through your case thoroughly and well understand your health issues. The symptoms seems like Otitis media and Otitis externa. In most cases symptoms resolve without incidents. However it seems like yours is being prolonged. I generally treat such patients with antibiotics too. I usually prescribe her with Azithromycin immediate release tablets 500mg once daily along with tablet Fexofenadine 120mg twice daily + Cortisporin solution 3 times a day (all for 8 days). Please note cortisporin drops should be avoided if there is an ear drum damage.

I am not sure what antibiotics was used earlier. But I am sure azithromycin will suit you. And if the pressure in the head + ear is troubling you, you can add 2 tylenol (acetaminophen).

Since your symptoms are resolving, I think you can postpone ENT visit for now. However consider the aforementioned treatment as well as blood sugar test. Keep your ears dry and clear. Avoid probing the ear. Plug your ear before a bath.

Thank you for your response. I am relieved to finally know what my symtons are and a possibility of being able to treat it with the mentioned Antibiotics...however I must ask if it is safe to take whilst pregnant as I am 7 weeks along?

Welcome back, and I am happy to answer your concerns.

I wish you a safe pregnancy and want to make a very little change in regimen. Fexofenadine is a class C in pregnancy category medications ( this means that use with caution if benefits overweigh the risks). As I know little about your pregnancy status so I will go for a better alternate, more safe and over the counter available Class B drug. Take Claritin 5mg once daily at night for 5days only, instead of taking Fexofenadine.

Your antibiotic Azithromycin is Class B, means its safer in pregnancy and breastfeeding. You may take it as guided in my last answer.

And the Cortisporin Otic (ear drops) is not absorbed in the blood in any significant amounts, so its totally safe for you and the baby. You may take Cortisporin as guided earlier.
