I have studies your history and the risk factors associated in your case and come to a few differential conclusions.
I believe you could be suffering from Osteoarthritis of the hips and knees. This is commonly seen in the older age group persons and I generally prescribe pain medication like Tablet Ibuprofen 200mg to 400mg twice a day after a meal.
Also I would recommend steroid or hyaluronate injections into the joint if the pain is unremitting even on oral medication. This combined with physiotherapy usually helps to limit some progression of the arthritis from getting worse. Steroid injection is very good for the pain relief. Hyalunorate offers some relief in moderate arthritis by helping the cartilage renew.
In the later stages, if the joints are damaged due to the arthritis, I would recommend joint replacement surgery.
Since you are not getting relief from Aspirin and Tylenol since some time, I would recommend you get steroid shots in your joints with cortisone. This shall provide relief for upto 3-6 months in some cases.
Also it is very important to follow certain precautions, to help gain some relief. Avoid cross leg sitting, squatting or kneeling, minimise climbing stairs. You can apply warm soaks around the painful areas as they improve local blood supply and help in pain relief. Also apply elastic knee caps over both your knees. Eat healthy. Keep well hydrated. Perform non-loading cardio exercises to help stay fit and prevent obesity which is also known to make arthritis worse.
