Suggest Treatment For Pain In The Big Toe

Question: I am 47yrs home maker. As a home maker I have to take care of both outside and at home works involving lot of walk. From last two months suffering with pain near and around Big Toe on left foot. General Physician prescribed blood and urine tests. All blood test reports normal. Uric acid levels within normal range so suggested to consult Ortho Dr. Upon consulting Ortho Dr. I was told that I am suffering with Osteophytes. After reviewing X-ray Dr mentioned that it indicates Hallux Regidus and three treatment procedures were suggested. 1. fusion, 2. replacement of joint and 3. grafting & cleaning of joint.

After coming back from Dr. consultation I have following questions

1. Which is the best solution to proceed for my problem? I need to take care of family consists of aged Mother in law, daughter who is in her 9th standard and my husband is a cardiac patient
2. Any other new procedures at present?
3. What is the recovery time for these procedures? Will I able to walk and attend my works as usual like before operation.
4. After this surgery is there any possibility of it reoccurring again in future?
5. Since Dr. mentioned severe arthritis, is there any chance of spreading this problem in the other joints or foot as well?

I would Prefer fusion.

Detailed Answer:
Hi I am Dr Gopal Goel Orthopedic Surgeon .
I have gone through the details and prescription.Arthritis of big toe joint is a common problem. In my clinic I start the treatment with pain killers , hot fomentation and local analgesic gel. If I do not get any relief then I go For Intra articular Steroid injection. As for your specific queries:
1.Out of three procedures ,at 47 , I would prefer arthrodesis ( Fusion) of the joint . It would provide pain free and stable joint. The time taken would be approx. 3 months but you will not be bed ridden.
2. There are no new procedures apart from these.
3. As I have written , approx. three months total for fusion , but walking allowed after some time ( 2-3 weeks).
4.After Fusion , unless the surgery fails there is no chance of recurrence.
5.This is local arthritis - it does not spread to other joints.

Thanks for quick feedback.

With respect to Fusion, it means that the joint will be screwed and plated with no union is it not ?
Recovery time is min of 12 weeks during which i would not be able to attend my daily house hold activities like cooking, washing clothes, house cleaning etc is it not?
As a part of rehabilitation i need to have physiotherapy is it not?
Normal walking will be difficult until 12 weeks is it not?

Fusion means unuin of two bones.

Hi
Fusion means joining two bones -there has to be union between two bones of the joint . Method of fusion will depend on the surgeon.
Normally after fusion I give Cast and my patient walks on the cast , may be after surgical swelling subsides - 2-3 weeks time.
You will be able to do part of activities in this time and I hope that you will be able to do all activities after 12 weeks. Up to 12 weeks you will have to adjust .
