I can get the following points from your history related to the testicular pains as described by you.
You have already been seen in ER, CT done, blood and urine tests and taken rounds of antibiotics and painkillers, best help is by Tylenol- OCD and Aspergers under medications. - most suspected is a nerve problem.
I do agree with your and your Doctors' views.
Since your investigations are normal, I would advise you the following::
It seems you have to take an opinion of a Urologist, who can examine you physically / clinically. This physical examination can give some diagnosis that no other tests can easily give for example varicocele or so; local nerve entrapment, creamaster muscle dysfunction or so. All these differential diagnosis can be ruled out; and your Urologist may advise you to have altered dosages of the medications you are taking.
Ultrasonography and color doppler examination.
MRI of the spine if advised by your Urologist.
Till then I would advise::
Scrotal support to lift the scrotum.
No straining like if you have constipation or coughing problem, get treated.
Smoking relieving the pain indicated towards Varicocele.
