Brief Answer:
Sureery is better option with scan says

Detailed Answer:
Hi,

You wife had pain in the right upper abdomen called right hypochondriac region. ON scan there is evidence of stones which are about 10mm in size. These stones are unlikely to reduce in size though moderate. One stone is in the neck and there is a chance of expulsion. If so them it can obstruct in the cystic duct and cause more pain as the duct is smaller than the stone.
Second the gall bladder wall is already distended that means it is trying to expel the stones out of it, thereby distended.
To avoid emergency surgery it is better to get elective (planned ) surgery. Emergency surgery can get to complicatons.
Now your wife has small hemangioma in the liver which might have different reasons because of pills etc. This should be considered as well and should not be punctured (unlikely if not growing) during surgery. So planned surgery is better.
Medicines have minor role here.

Hope my answer convinces you.
