Brief Answer:
It may get complicated.

Detailed Answer:
Hi
As per your description, hernia is present since 20 years.
It is the passage of bowel or omentum (abdominal fat)which causes the swelling.
If the bowel gets blocked, there will be pain.Later, it may lead to distension of abdomen and vomiting.
There is no medical treatment for hernia.
Please consult your Surgeon and get examined clinically.
If it is obstructed, then he should be operated.
I have seen many patients who presented after complications. If the blood supply of the bowel is stopped due to severe obstruction, the bowel may become gangrenous. He has to undergo major surgery then.
Please consult your Surgeon immediately.
Nothing to worry.
If you have any questions, I will clarify further.
Wish him good health.
Regards
