Posted on Thu, 12 Nov 2015
Question: Hi, I have a painful lump on my calf that came on suddenly after no injury or trauma. There is no bruising or marks, it feels about half a cm under the fatty area and feels about 2x2cm, and not exactly circular. I get achey calf pain that is constant or when walking, but to press on the area itself is very painful.

Brief Answer:
Needs clinical evaluation and investigations.

Detailed Answer:
Hi.
Thanks for your query and an elucidate history.

I would like to know a few more details for me to assist you better:
- When has the lump occurred?
- Does the swelling increase on contracting the calf muscles?
- Can the swelling be reducible on relaxing the muscles and pressing in?
- Is the swelling movable or fixed to calf muscles?

Sudden appearance of a tender and painful lump on the calf without any obvious injury or trauma, without bruising or marks, deep within and of the size of 2 by 2 cm which is is not exactly circular can be due to the following reasons:
Infection
Varicose vein entrapment
Hernia of the calf muscle

Obviously this needs a clinical evaluation, actual examination by a Doctor/Surgeon and investigations like high resolution ultrasonography and color doppler machine to know the exact cause and to get an appropriate further management.

I hope this answer helps you to get an early and proper diagnosis, please feel free to ask for further queries if you need to or if you feel that there is a gap of communication.

The lump does feel reducable (or perhaps just not firm), it does not stay reduced/ reappear on contraction it also does not increase in size on contraction. I do have mutiple varicose veins on both legs. Is this something i need to have investigated as soon as possible, or can i wait to see if the pain resolves? The actual pain and lump came on suddenly overnight. Perhaps i noticed some calf pain yesterday, but i only palpated the area and found this lump this morning.
