Brief Answer:
May indicate prostate issue

Detailed Answer:
Hi and welcome to HCM.
In first case it indicates prostate problem such as hyperplasia or prostatitis and it can occur with no obvious reason but also may be sign of STD as well. You should visit urologist, do prostate ultrasound and have urine analysis to rule this out. other possible causes are less common and include herpes infection, hemorrhoids, perianal abscesses or fistulas. If prostate issue is ruled out then you need to see proctologist.
WIsh you good health. Regards
