Brief Answer:
Could be a Calcified Plaque

Detailed Answer:
Hello
Thanks for your query ,based on the facts that you have posted it appears that you have noticed a painful lump in shaft of your penis since one year .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Is the lump (Mole ) hard on palpation ?
2) Is it free from skin ?
3) Is it fixed to deeper fascia?
4) Does it cause pain on erection or while attempting penetrative sex ?

The photograph that you have uploaded is shoot up from very close angle hence can not be visualized properly If possible take a fresh photograph from adequate distance focusing on the lesion and post it again.

From the description that you have narrated It could be what is called as calcified Plaque or a subcutaneous soft tissue mass and can be confirmed only on clinical examination .

Please consult qualified Urologist for clinical examination and get Ultrasound scanning of Penis done to confirm the diagnosis.

Further treatment will depend upon result of these tests and final diagnosis.

Hope I have answered your query ,please feel free to ask if you have more questions ,I shall be happy to help you
Thanks and Regards.

I took a better picture, the roughness is not hooked to the skin, its on the muscle, only on the side the mole is,it becomes more tender when I have a erection.
