default
Posted on Fri, 16 Dec 2016
Question: Have been experiencing heartburn issues for about a year and a half for which I take nexium occassionaly. Noticed over couple of months I have bouts of nausea and cramping of the upper stomach region. Ate a doughnut and drank some milk last night late and immediately after felt as though right at the base of my sternum was on fire, bloated or something. took two tums and 2 advil. After about 30 minutes the pain started to decrease but still there. It has now been approximately 8 hours and still having pain in the same region, though not as bad. No other symptoms such as fever, nausea, vomiting or diarrhea. The rest of my body feels great, just this one area.

Detailed Answer:
Hello and Welcome

I appreciate your concern

The symptoms you described and the history is classic for GERD (Gastroesohagel Reflux disease ).

NSAIDs like ibuprofen (Advil) can worsen GERD and GERD related erosion / gastritis , so they should be avoided.

PPIs like lansoprazole can be advised at optimum dose e.g 40 mg once a day initially and can be increased to twice a day for severe reflux disease. A gastro kinetic like itopride should ideally be added to the regimen. The treatment should last for at least two weeks. If there is no or little improvement despite these medication then further workup for other causes including an upper GI endoscopy can be advised after due consultation with your doctor.

Wishing you best of health

Thanks
