Greetings!

Problem : I am having very oily skin along with small pimples & black marks with small hairs.
Give me suggestions for oily skin along with removal of small pimples, black marks & hairs permanently

I have gone through your query and I have understood it.

Oily skin and acne go hand in hand. Treatment of acne is guided by the severity of acne and includes both topical remedies and Oral forms of treatment.

Acne lesions can be of different types i.e comedones, papules, pustules and nodules. Patients with acne usually have combination of these different types of lesions. Papules, pustules, nodules etc are inflammatory acne lesions and are usually seen in moderate to severe grade of acne. Papules, pustules etc are also prone to leave behind black spots and sometimes scars.

I usually ask my patients of oily skin to use a salicylic acid based cleanser e.g Neutrogena Oil free cleanser, twice daily.
In addition If my patients have inflammatory acne lesions like papules, pustules etc I ask them to use Clindamycin 1% gel Or Benzoyl peroxide 2.5% gel during day time. For night time application I would normally suggest Adapalene 0.1% gel, to be applied over the whole of acne prone area i.e usualy cheeks and forehead. Adapalene is coemdolytic and therefore prevents new acne as well as cures existing ones. Adapalene also helps lightens acne marks/ spots.

In addition to topical treatment I also start my patients of inflammatory acne lesions (i.e papules, pustules etc) on Oral antiacne antibacterials e.g Tab. Doxycycline 100 mg once or twice daily. Oral antibiotics may be continued for 2-4 weeks or even longer depending on response and severity of acne.

Facial hair and acne are signs of hormonal imbalance and this needs to be rules out on history and lab tests. Since you have a problem of facial hair, therefore I would also like to know whether do you have any irregularity in menstural cycles?

For coarse facial hair I would suggest you to take sessions of Laser (Diode Laser). Prior to taking laser it is important to rule out any underlying hormonal imbalance ( usually suggested by irregular menstural cycles) and lab tests for hormones i.e Testosterone levels, DHEAS levels, LH/ FSH, Prolactin etc.

I would suggest that you visit a doctor in your vicnity for a visual assessment of the severity of your acne. Your doctor will also try to rule out underlying hotmonal abnormality on history and might also order a few blood tests for hormones. If there is suspicion of hormonal imbalance on history and lab test then treatment with monthly hormonal pills may be started.

Dietary advice for acne

Detailed Answer:
Hi.

Avoid high calorie foods like chocolates, sweets, coke, pizzas and pasta etc.
These are high glycemic foods i.e promote higher insulin levels to maintain blood glucose levels and thus insulin resistance, which in turn cause more of circulating androgens and acne. Including more of low glycemic foods items like green leafy vegetables, salads, fruits, whole grains and pulses is beneficial for acne.
Metformin e.g Glycomet-SR tablet, increases peripheral insulin sensitivity i.e reduces insulin resistance, therefore, lower insulin levels are required to maintain blood glucose levels and hence benefits acne due to insulin resistance. Therefore, glycomet/ metformin can also be started in females with acne and possible underlying hormonal imbalance. You can discuss this with yur treating doctor.

Drink at least 10-12 glasses of water everyday. Maximum in the morning.

I won't suggest any ayurvedic treatment for acne.
