Suggest Treatment For Post-inflammatory Pigmentation In The Groin Area
Question: Hi doc,
I am a 25 year old male. XXXXXXX origins fair complexion.
I was obese before and used to do lot a cardio. I think I sweat a lot too. Might have gotten a groin fungal infection in the past but nothing recently. For the past 6 years or so. Area around my groin has become darkened. Inner thigh area. I think this is a friction and post fungal hyper pigmentation. As I lost weight the area of hyperpigmentation. Has decreased but in the areas where thighs meet just below the testicles it is still dark. As I am keeping the area dry it is becoming mildly lighter. But still I really like this girl I met and I a too embarrassed and it is affecting my sex life. Any suggestions should I use any topical cream. Am a medicine resident so too embarrassed to go to the derm clinic where other residents will be there. Should I use any cream? Thanks

i suggest you to use a mild topical steroid+antifungal cream combination e.g clotrin- B cream Or candid- B cream, twice daily for 2 weeks. The antifungal would take care of any persistent fungal infection and the mild topical steroid would take care of post inflammatory pigmentation.

Thanks for the quick follow up. I am confident there is no active fungal infection at this moment. But for hyperpigmentation secondary to friction and sweating. Is a steroid cream the best bet to reduce skin hyoerpigmentation. Just asking out of curiosity. What about something like hhdroquionine or melaquin? I was trying to understand the concept behind why there would be skin hyper pigmentation from skin friction and sweating and if this is common

hydroquinone or melaquin would be too irritating over delicate groin skin. A mild topical steroid would work ideally in case of pigmentation which is most likely post inflammatory hyperpigmentation due to past fungal infection Or friction, rubbing which induces skin lichenification and darkening Or due to acanthosis nigricans.
