Suggest Treatment For Profuse Sweating, Heart Palpitation And Depression

Question: I am an XXXXXXX working in Dubai from 1992.In 2000 when i went to XXXXXXX for my annual leave i had a strange feeling.I suddenly wake up and had severe fear,sweating and palpitations.I could understand it and not sleep well.Later it has not repeated on my vacation.When I returned from XXXXXXX and reached in dubai first day itself i had the same feeling of intense fear.i couldn't relax soon but very late i went sleep.few days while i was watching tv i had sudden fear and feeling iam going crazy,Then continously thinking about what happened to me i started have the fear,sweating and palpitations more severe.Slowly I become depressed and started to work like a machine.Some times feels depressed at the same time i feels agitated.Slowly i lost sleep and lost the interest in life.it continued for one month
and i had shared the incident to my friend and he referred me to a neurolist.he given me some xanax tablet and i got my sleep back but the other symptoms remains same.Later i met a pschiatrist and he prescribed medicine for depression.later doctor identifed as bipolar mania due to my hyperactivity,over enthusiasm and sleeplessness.he has given me medications lithium 400 mg(2 times),lamitrogine 100(2 times),cipralex 5mg(morning).I was taking this medicine more than 2 years and feeling good.last month i had severe manic episodes and doctor stops all the medicines.after few days my panic attacks has returned.Doctor prescribed the same medicine but it wont work well.Then he provided me rivotril .5 mg(2 times).But I didnt take this medicine as the doctor told me it will cause dependency.But last two days i couldnt sleep due to racing thoughts and anxiety. I feel tired but unable to sleep.Due to severe anxiety i will go out for a walk in the park even in mid night.

Please tell me whether I can go ahead and take rivotril for a long term.
you can prescribe any other medicine apart from prescribed by doctor.
Shall i need to change the doctor.

Thanks for writing to us.

You have history of panic disorder, depression as well as bipolar disorder. All have been treated well.
At present you have anxiety problem. Rivotril (clonazepam) is the best medicine for anxiety as per my clinical experience and most of the psychiatrists use it as first choice for immediate relief in anxiety.
You can take it as per your doctor's advice. It has long half life as well as less relatively less potency. It has almost very low potential of dependence among benzodiazepines. You can of course stop it after complete improvement through tapering under your doctor's guidance. Do not hesitate to take it.

Yoga and meditation can also help.
Use of Deep breathing technique (slowly inhale for 3 seconds deep in lungs with open mouth, keep breath in lungs for 3 seconds and then exhale slowly for 3 seconds) while panic attack can reduce/stop severity of symptoms.
