Suggest Treatment For Prostate Cancer

Question: Hello Doctor, I've been diagnosed with tc1 prostate cancer with 4 of the 12 needle biopsy samples malignant and given a XXXXXXX score 6, PTI 5,5,5 & 20%. Three of the four malignant areas appear to be on the periphery in a 2D view of the Prostate I have an 84% chance the cancer is contained in the Prostate. (See the following 2 uploaded documents for only the pathology report. Sorry, I don't have a scanner). Apart from this ailment, I am healthy, active sexually and physically. I currently work in the Oilfield on international assignments. I am 62 yrs "young" and married.
My dilemma is how do I treat with the present situation for the best outcome. At this point I am considering 3 options. (a) Radical Prostatectomy with Robotic Surgery (b) Focal Laser Ablation (c) Radiation. (in which RA seeds are planted into the Prostate for treatment (ProstRcision)). I do not know of any alternative.
I ask for help to make a decision. Can you assist ?

Brief Answer:
Hello dear. Surgery is a good option.

Detailed Answer:
Hello dear. I have gone through the details. So you have a low volume localised cancer prostate.
As you have a good life expectancy, low XXXXXXX and fewer core positives with localised disease, surgery in expert hands will be good option.
You can go for radiation treatment too.
Focal laser ablation is not recommended due to high chances of relapse.
Discuss these options with your doctor too.
Surgery will have best results.
Thanks and regards

Hi Doctor,

Thanks much for your prompt response ... much appreciated. I understand your mail.

I can get scans of the 2 page pathology report which I've attempted to describe earlier. The contents might not change your impression but I can upload them if needed.

Our unanimous opinion at this time is surgical removal of the gland with robotic technology manned by an "experienced" doctor at the Global Robotics Institute here in Florida.

Our only concern is the ~16% chance the cancer is not completely contained in the Prostate which I guess would be the same situation regardless of the method used for treatment but we are confident this can be handled by our surgeon with whom we have an appointment. We will certainly discuss this aspect as well.

Doctor XXXXXXX thanks very much for everything. I will leave this discussion as "open" and perhaps allow it to close automatically in case I have something further on the topic.

You take care ...

Sincerely, XXXXXXX
