It is possible that the achy discomfort in lower abdomen and testes is unrelated to the rash/red spots on glans.
The red spots are possibly due to yeast infection or irritation to soap etc given that you have tested negative for STDs
I suggest you to take an oral antifungal e.g Cap. Itraconazole 200mg (Cap. Itrasys 200mg) once daily for a week.
Topically you may use an antifungal cream e.g Candid B cream, twice daily for a week.
Wash the area with just plain lukewarm water. Avoid soap etc.
Regarding lower abdomen and testicular discomfort you may also take an opinion from a surgeon.
