I am Dr. Prakash HM and I will be answering your query.
Based on your query, my opinion is as follows:
1. Increased risk of repeated urinary tract infection, present due to bladder stasis.
2. Limb swelling and pedal edema, can be reduced by limb elevation in lying down position, above heart level, with 2-3 pillows below legs. Continuing limb edema can cause ulceration and increased risk of infection.
3. Being bed ridden is not good psychologically, caretaker support and movement will be helpful.
4. Cranberry juice and blue berry juice is known to reduce risk of bladder infection. Take cranberry in particular in limited quantities.
5. Take egg white to improve protein levels and also to build immune system to fight infection.
Continue followup for controlling diabetes and hypertension. Dialysis will be dependent on gomerular filtration rate. Creatinine 1.3 - at present would not be necessary. Creatinine more than 2.0 with gfr <15%, dialysis would be necessary.
Would need nutritional support, family support and involvement in social activities to improve psychologically and physically. Physiotherapy is essential.
