Question: Hi,
A close family member is suffering from major heart problems and have been recommended heart surgery for the same. I would like a third opinion on it as it is a major step and I plan to cover all bases.
I have all required reports with me in soft-copy format, please let me know the email id I can send them to.
After going through the required documents, please let me know the type of surgery (if required), where it can be done and what should our next step be.

Dear sir

As per the ECHO report and notes of the doctor attached, patient has rheumatic fever with rheumatic heart disease, severe mitral valve regurgitation with severe pulmonary artery hypertension with good LV systolic function.

1. The surgery in this case primarily depends upon the symptoms of the patient. If patient has symptoms of the disease, surgery should be done. The usual symptoms of this disease are shortness of breath on exertion & easy fatigability. However presence of severe pulmonary artery hypertension is one of the secondary indications of surgery, which is present in this case.
2. The primary surgery in this case will be replacement of one of the valves of heart i.e. mitral valve. However additional procedure may be required which may be coronary artery bypass surgery and / or tricuspid valve annuloplasty.
3. This patient will also be subjected to coronary angiogram before the surgery.
4. It can be done at any big hospital which has the facility of cardiac surgery.
5. Next step is to consult a good qualified cardiologist at a center which has the facility of cardiac surgery also.
